I'm suprised this wasn't already said but mine is... Someone picks nighthound and waits for the last second to pounce and get the killsteal .. sigh

When your in a lane and your support doesn't know how creep agro works so every time they attack a hero they pull the creeps into a tower or right as I'm about to get a last hit so I miss it.

when people play tb and dont open with ult to A) inflict a shit load of dmg (which usually most doesn't count because it does 300 and they have 100 hp left (hence doing it early)), catch the enemy team off gaurd, C) show locations of everyone, D) Scare the hell out of them, E) genocide.
