items drop on death in hardcore, it actually specifies that in it's description as well as on patch notes. It's a novelty mode that rewards carries and heros that can ez creep waves, junglie heros like zepher and warbeast will basically be able to solo teams after 10 minutes of farming
win early game in hardcore = win the whole game ^^
