repetition
logical appeals (why were you forced to do it?)
emotional appeals (i was really having a rough day, i was too tired to deal with it)
use words that exceed the vocabulary of GMs (6th grade +)
write a hefty paragraph, 9-10 sentences.
don't suck up, but also don't call the gm a retard (suggest that maybe the reviewing gm didn't watch the replay to get the whole picture)
rhetorical questioning can work too
successful appeal on a game i rooped after my team made me mad:

I don't quite understand this ruling. Granted, I played badly and died a lot - but I was tired, and under a lot of pressure from my team that talked trash to me from the beginning. I died a cheesy death to Salforis, giving him an early advantage, and a pollywog came mid and did basically nothing. I wanted to towerdive salforis, tried to get him to come, but he just sat there getting cs. So I ended up dying again. And then it just snowballed, because I had no items and their team had teamplay. This was also an extremely quick ruling - I highly doubt the GM even spent more than 2 minutes on this, simply looked at my deaths and assumed I was griefing. What? I didn't even have the most deaths, my 20 deaths to polly's 21. I also had the 2nd highest creep kills. I didn't do too bad in assists, or kills. I just don't get why I get banned for griefing when I wasn't, I was just playing improperly? Should I only play when I am at my mental peak? Should I report the pollywog for feeding, and should he get a heftier suspension since he died more? Should I report blue for using an obviously overpowered early game hero? I just don't get it. Thanks
obviously something exactly like this won't work since yours wasn't for griefing, but you know, model it somewhat like this
