there‘s a node in the breach atlas tree which gives you a 2% chance for 10 additional breaches.
i think it‘s impossible to get more than 2 or 3 without that node.

I actually just had my first 12 breach map spawn yesterday, it was great.

Wish I could up that. It was nice getting 400 breach shards from it, and then going do that as well, lol.
