The point is just that obesity is normalized to the point where 250lb/6ft is considered just "husky", and 'obesity' as a term is distanced from its actual medical definition and commonly thought of more as what is superobesity.
I think you'd know if you're so jacked that your bodyfat is sitting at 20% even though you're sitting at 220-240 pounds.
BMI is a weight approximation that does not take lean body mass or muscle into account.
Even then the golden rule is that you're always carrying more fat than you think you are.
