People's subjective experiences are biased as fuck and they're not always the most reliable source of information even when it comes to mental health disorders which are often only of subjective nature when mild. People are surprisingly often wrong about what is actually going on in their heads. Not all depressed people are as depressed as they think they are, and people are often quite poor at questioning their own experiences and go defensive when someone else does it for them.

Everything is arbitrary, the concept of biological sex is just much more convenient. Not all are clearly male or female, but most fit in either of the two categories with the rest falling in between. We just accept that it's the most practical solution even in its imperfection
