Smoking definitely causes emphysema...not so much lung cancer.
What? How does smoking cause emphysema but not lung cancer? Also, if smoking caused emphysema (your words), how does smoking not kill? Emphysema kills.
Also, your analogy of saying "Saying that smoking kills is akin to saying vaccines cause autism" is extremely faulty and a misunderstanding of the correlation-causation concept. Relative risk describes the strength of relationships in populations.
There IS an association between smoking and negative health outcomes (that lead to death), but there IS not association between vaccines and autism. Saying smoking kills is definitely not the same thing as saying vaccines cause autism.
