If the "history" of the situation had been different, they'd be living short, violent lives without the benefit of modern medicine. That's not to say that Europeans were much different before the advent of more modern and robust institutions, it's just the reality of premodern life.
That's not exactly an accurate portrayal of the state of civilization prior to European arrival.
They had society, large cities, institutions, etc. etc.
The big advantage Europeans had was that the American population was largely wiped out by pandemic (brought by the vikings) about a hundred odd years prior to the European arrival. If history had been slightly different (if the Americas had more than one or two domesticatable animals) Europe would have been wiped out by "Americapox" going the reverse direction.
