I honestly don't understand what's wrong with the latter? Like who else is supposed to be responsible for a child's health? Why is the school asking parents to be responsible and monitor symptoms and isolate their children a controversy?

well, one reason is school isn't optional, and you can be prosecuted for not sending your children to school. i know, i know, homeschooling is an option. but still.

From what i understand this form is about giving parents a choice if their kids are going to participate in sporting or whatever events in light of covid. We know Covid is a disease that doesn't affect kids and you could count the number of deaths of kids worldwide on your hand, so not sure why this bitch is thinking something outrageous is happening here.

A kid dying from playing football in school is about a million times more likely than he/she dying from Covid. Like this is a pretty standard release form for kids sports, field trips, etc. Why are so many liabilities in life being pushed away from the individual/parents to society?

nice try, but the 'controversy' is in forcing schools to open in the state of a pandemic that requires things like that.

should people go back to work / school / normal lives again? absolutely, literally no one WANTS to keep the country closed for any longer than necessary, but is that time now in america, with case numbers skyrocketing (at least until propaganda measures, like slowing down testing, and censoring the official numbers come into effect), and those who can financially and logistically afford it (and not trying to score political points) smartly adhering to distancing and protection measures? well, quite obviously not.
