I've watched a ton of procedures, both in the ER and multiple-hours long open heart procedures, and nobody in the room changed their mask a single time, even after 4 hours. It's a way greater risk to change your mask than it is to keep the same mask on, because the mask getting dirty from you breathing doesn't introduce a lot of risk as long as you aren't touching it.
If you are going to adjust your mask, remove your mask, etc. you should be using hand sanitizer afterwards, and this has been part of the mask recommendations since day 1.
