I'm sure where the guides bring you in Pyongyang is about the best conditions in the whole country, but from what i've read the poverty is inescapable these days.
He had about ~2 interactions where he thought people were being genuine. One was when they went to the beach and people were there who weren't supposed to be there (national holiday). Guides freaked but the tourists just chilled with the locals a bit. They didn't know what a high five was.
Other one was with the guide, after a LOT of alcohol. Which btw is one of the things you'll do there, as outside of the guided tours, you're just stuck in places that have nothing to do but drink alcohol.
