It does matter when such immoral behavior is coming from the candidate trying to run on a "return to normalcy, decency and morals".
This episode is the death blow for the key argument he has against Trump. It exposes him as a hypocrite and takes away the credibility from the only platform on which he could effectively campaign against Trump.

From the point of view of a cookie-cutter conservative, Trump has done well in terms of judicial appointments, abortion, gun rights, tax cuts, defense budget, deregulation and most aspects of the economy.
The only avenues of attack against him in a Republican primary are "lack of morals/basic decency", "protectionist" and "too much government spending".

The protectionism is only of concern for chamber of commerce republicans, and they just are not a numerically strong faction within the party.
The deficit only concerns some hardcore activists and the donor class who use it as a vessel to push for their libertarian agenda. The rank and file never cared about the deficit per se, they only care about it when it's a president form the other side piling up the debt.

This leaves Trump's erratic and crude behavior as the major weakness in the context of a primary. And to launch effective attacks on this weak spot, a primary opponent needs to have a better personal track record than Trump on this front. Which Sanford just does not have.

You're assuming a false dichotomy. There is more than enough space on the political spectrum between "down syndrome sjws" and "trumpism".
