A plethora of reasons. From a brief respite, to the alleviating of symptoms, to hatred towards one self, lack of distress tolerance in general, or fundamentally not liking the way one interacts with the world at baseline, and most importantly drugs just feel good and have been part of our mammalian life long before we were human. Goats will grind down their teeth in psychedelic lichens and every animal has a way it likes to get fucked up. Humans are animals and based on chemistry alone we will have a relationship to drugs.
Also social factors, who is giving said drug will categorize it into drug or medication etc and that is all culturally constructed leading to more obfuscation of etiology.
Marijuana has been an illegal street drug with no medical value until now...so what in the plant changed? Hah.
