Melbourne in Australia used to be rather clean when I was studying there.
Auckland in New Zealand is pretty clean.

I think communist countries might not be exactly that clean but the draconian laws might do some wonders at times.

Even when I used to go to school back east we would have thousands of ladybugs each year in the house. Ants always no matter how clean.

I live in a fairly small 150k central european city and its not gross anywhere here afaik. Lots of tourists seem to like it here as well, especially Americans and Asians.

So no, not every city is gross.
