It happened in one particular neighborhood (Woodland Hills) and was caused by provisional ballots being mailed. The mistake was quickly recognized and corrected. Again, you are talking a specific neighborhood in one county of California.

fraud is already documented.
it appears the ballots were filled out in advance. the stuff was all done electronically. people went into vote and were told they voted already.
