Back in 2009 a study in the American Journal of Public Health found that some 45000 people a year die just because they can't afford healthcare in the US. I can't imagine that number has fallen in the last decade either considering the widening wealth gap.

because "gender" is not the same as "sex". there are biological differences between men and women, and they also affect their gender perceptions and inclinations, but not to the degree of predetermining them entirely. thus, you have a situation where 95 or so percent of people are comfortable assuming the gender role that matches their biological sex, but some deviations exist as well. Transgender persons are persons who are a full male or female on a biological level, but their social gender doesnt match with their biological sex. And then, you have the very rare cases of transsexual persons who are something in between man and woman on a biological/physical level.
