On a scale from 1 to 100:
Phelps: a 97/100 on skill, then gets +2 from his genetic condition. A small boost in absolute terms, but in a professionalized sport where he is racing against lots of other athletes who score in the 94-98 range, those extra 2 points make a difference and disproportionately translate into medals and world records. But this advantage he had was small enough that subsequent generations of swimmers could quickly overcome it due to advances in sports science and take away his WRs.
Khelif: a 35/100 on skill, gets +30 from being an intersex person with male levels of testosterone. A huge boost in absolute terms. In a not very professionalized sport, Khelif faces lots of thoroughly mediocre opponents who are overpowered by her male advantage. Still doesn't translate to a lot of titles because Khelif still only reaches a 65/100 with all of the advantage and is still very beatable for the few top athletes in the field.
