Metro Honolulu has a population of 1m and is quite dense in large parts. Definitely a big city.

Rates of every sort of violent crime are lower in Japan than the U.S. by several magnitudes. Firearm access cant explain that. Likewise, places like Switzerland or Finland have rates of private gun ownership comparable to that of the U.S. yet significantly less victims of gunshots per capita.
The tldr is pretty easy and obvious: local culture, social safety nets, strong communities, drug usage and gun accessibility are all important factors influencing the rate of gun violence in a country. The United States suffer from a particularly toxic combination of these factors: a culture glorifying violence, big poverty and inequality, a weak social security system, lots of addicts and many disrupted, dysfunctional communities are combined with super easy access to firearms. The unsurprising result are shooting sprees every other week. Tackling just one or two of these factors will mitigate, but not really solve America's problem with gun violence.
