I looked into Klinefelter syndrome, it's very interesting and I see they are particularly susceptible to all sorts of ailments, but I fail to see why you think it ought to be classified as a third sex. Why do you think it should be?
I think the pronouns addition may have solved that to a large degree.
If god forbid they need surgery, the doctor will need to know their birth gender in order to properly dose things like anesthesia. Those with Klinefelter have traits that don't match man or woman.
However, they can also choose whatever pronouns they want in any other case.
