The person on the bottom of those two memes is a normal looking person with probably no health concerns. I wouldn't see her and be like "metabolic syndrome".
It is uncommon to see a really really obese person put a picture of themselves in their bra and panties and people be encouraging. Maybe you can find a few. I mean obese, not somebody who enjoys three meals and day and light exercise like the woman on bottom probably does. This girl doesn't even have a gut.
magazine covers are supposed to be motivational showing what healthy bodies look like
the obese insecure fatties have grown large in number and have started swinging their weight around to change societal values and they've successfully normalized obesity which is why you see a proud obese woman like that as normal
i wouldn't be surprised if she had knee problems that can be attributed to her weight

the things you're allowed to say in public without any kind of repercussions is a perfect representation of things that are socially acceptable
conversely a number of groups that have been advocating for healthy fit bodies have been banned from various kinds of social media outlets because their message of promoting healthy values has been labelled hate speech
