Society as a whole is too focused on body image tbh. I can eat like shit (barely any fruit/veg) and still have a pretty nice body. Doesn't mean I'm that healthy. The focus should lie on making sure people have access to healthy food and a kitchen, and know how to cook. I'm hanging out with quite a few undergrad students right now and quite a few of them don't have any cooking knowledge beyond baking an egg.

In a way I think all of these obese people are perfectly aware of their weight problems and that it's not healthy. They probably have just given up because a lot of weight loss programs are scams and they see no success with them. Whatever fad diet is being made, it is made so that people try it for a short time and gain the weight back so they can go use the same or another program again.

Also note that being active == healthy/rich is a relatively new trend too. Even in the latter half of the previous century you were rich when you did not have to do manual labor. Some of that might still be dwelling around in society.

Solutions to the problem:
- Eliminate food deserts
- Mandatory PE in schools (primary and secondary education)
- Making sure that people have access to a kitchen (whether it's shared or not does not matter much)
- Cut down on corn subsidies and spend that money to subsidize vegetables/fruit.

If their not sufficiently shamed by their claim there probably isn't a way to shame them enough or ever rehabilitate them to the point they become contributing member's of society in even the slightest imaginable way.
