This gets at it, but if we're talking about 1000 years ago, what would someone say?
Someone 1000 years ago would know if someone was a woman by their appearance, voice, etc. If they discovered this person had a penis or ambiguous genitalia, they would conclude they weren't a woman.
Sure, there could be an instance of someone who looked and sounded like a man, but was a female.
But I don't understand why it's necessary to upend the gender binary for a statistically miniscule number of people. There are humans who defy the norm, but I don't see why obliterating the norm is a reasonable response to that. Generally, human beings fall into two categories, male and female. There's anomalies, but that's the overwhelming reality for human existence. And these men and women can express themselves differently, but at the end of the day, they are men and women.
Edit - Reading my post, and I may be conflating the sexual binary and gender binary, but I'm not entirely sure anything I've said is wrong.
