Bush Sr. was a foreign policy success, but a failure on domestic policies and the economy.

Probably James Buchan. Dude refused to challenge the spread of slavery, fucked up the 'Dred Scott' case, & basically left the Union in such a fucked up spot, that the Civil War soon followed.

I think a case can be made for Andrew Johnson. He led an extremely corrupt administration and deliberately sabotaged reconstruction. Without him, there's a chance for lasting social progress in the South and for blacks to not suffer through a century of Jim Crow. A more successful reconstruction would also have had endless amounts of knock-on effects. A South which uses the potential of its black residents instead of wasting resources to maintain a regime of social and economic apartheid would have decreased regional polarization in U.S. politics to this day and prevented a lot of bad politicians and even presidents from coming to power.
Also, while Buchanan was clearly a failure, I think one can argue that the Civil War had already become inevitable by the time he took office. A different guy in his shoes could have delayed the Civil War by another couple of years, but I'm not sure anyone could have averted it by that point.
