Almost everyone I know that has a Ph.D. doesn't use the title. I've heard people with PhDs actually say don't call me a doctor. I think it's a bit pretentious and most of these smart people realize that, that's why they yield the title.

At work The Doctor means the MD for sure. You're right.

If I went further I would have to choose a PhD program or DSW program. Either way I would be a doctor, but I would just be Tom in most capacities. But I could turn the title On if I earned it.

I've never heard of someone with a Ph.D. actually responding "I'm a doctor" when someone asks for a doctor in a medical situation. As far as I can tell, that's just an internet meme. In any case, people who are looking for experts in a particular field often do look for doctors in that profession. This is standard when looking for expert witnesses in court cases, for example. It's all situational.

A podiatrist might not be useful in any situation where someone is yelling for a doc.
