This might be an oversimplification as im just reading up on this but it looks like the regulations that could have prevented this were opposed by rail lobbyist influencing a republican government under trump who repealed the requirements for better brakes on trains carrying hazardous chemicals

certainly some conjecture involved, but its still awful how such a large industry cant find a few pennies to make trains safer

not suprising though, poor handling of chemicals is not an exception, its the norm in most countries/places (tianjin, beirut most recent big disasters
