I could have done it myself, but I guess if I am asking if anyone has a similar template in mind here, it probably means that either I don't want to do it myself, or I just want to know if someone here has seen a similar template somewhere. Also, your link points to the graphic request forum, my question is simply a question, not a request for a graphic job. So, have you seen a similar template somewhere? Or do you just want to up your posts count?
