Who is the demographic? What do the elements in the picture mean beyond portraying depression? It looks pretty abrasive and graphic. Not all postpartum leads to the mother killing the kid. The outfit and hair of the woman kind of makes her look young, she's kind of ambiguous and doll like.
Reduce the number of fonts to one, two if you count bold as a second font. Legibility is lost when the text overlaps certain parts of the background. Trying adding a subtle outer glow and/or stroke.. or you could resize and crop so the text isn't overlapping. The negative space around the next could benefit from some spacing. Also, I would lower the font size, rather than track the orange text so much. It looks very pinched. Take some time to kern individual letters that don't mesh well.
There needs to be some negative space between the edge of her head and the edge of the paper, or you need to crop more of her head off. That tangent created by touching at the tip of her head is awkward. You did a good job cropping her arm though.
If you're making this for a portfolio, then I recommend trying to apply whatever it is you're going for to other elements, such as mailers, flyers, etc, to portray a campaign.
