Hey guys!

I'm planning on making a Graphic Request later on today, but I had some issues I needed to sort out first, a few questions.

I'm thinking of starting to Stream Starcraft 2 abit more just for fun and I want a neat overlay for my streaming.
When making this request, is there anything I need to think about?

Can any artist make one? Do I need to provide with a template for it? I tried to find some blank templates, but I couldn't seem to anywhere. (if you have one, I would very much appriciate if you could send it).
If I cant find a template, how do I know the correct measures for it? I will be using 1920x1080 res, thats as much info I could provide. Not really an artist
What would I expect to pay for this? Between 200-500? 500-1000? 1000-1500? 1500+?
