3ds max
blender (free)
maya

render a shape of your desire, do some lighting, test renders in 360p
put your design as a material matte
I know how to do this, but it takes a bit of time

If nothing else works its just making a plane and uving it with a color texture for basics. Can add texture with bump maps.

Getting into maya, max, etc is just overkill, it takes quite awhile to get use to with no 3d experience.

Cover Action Pro that was linked is by far the best for what you need.

If you actually want something more complex like that photograph, I'd suggest making a cheap lightbox and building what you want with real materials, then take a picture.
Put a temporary card in there and use scaling/perspective in photoshop to match cards to the layout. Then just match the light with a simple gradient.

Or even print out the real card.

You probably have everything to make a lightbox lying around.

Yeah cover action pro is really helpful, however I do believe photography may need to be used in order to achieve EXACTLY what I want >_<

If you give it a shot, I would like to see how it turns out :D

It's really easy to make, and the photos will look great with just about any camera.
