Maybe parts from photo weren't well defined so he couldn't see it well? Or he is very bad at drawing. How can you forget to draw a finger to a hand? I remember when i used to trace and i remember exactly how the result looked. I never got outline right because i wasn't concentrating to drawing rather than to a photo i was tracing. That's why i also forgot parts sometimes if i couldn't see them well from picture.
