l/q usually is either from a lack of finishing touches (blurring / sharpening in foreground / background respectively) or from using low resolution stocks and effects
often times c4ds and such are quite sharp or over sharpened, so further sharpening makes them look l/q
sometimes it's best to sharpen and blur individual layers rather than waiting until the last steps where you apply image
