there are professional artist that do this kind of thing for a living,
they're called tattoo artists and you can find them at your local tattoo parlor

I love his work, I just think he rushes the design part too much because he loves tattooing.
He's a family friend and I won't let anyone buy him work on me.

Family friend or not, I would not let anyone put anything permanent on my body, especially if they rush through the design process, people that rush through anything like this suck in general, maybe he will rush through the sanitation process and you will get aids, hepatitis C or some other fucked up disease. Just saying get a pro to do this and never go to a video game forum for this kind of advice, that's just stupid.
