Not necessarily, blizzard (not 100% sure, but pretty darned sure) has the transmute function query the server for the result. So the server may have all possible variations, and the client be clueless, other than that there exist some undefined permutations of the horadric staff, such as this one.

Also the reason people can't just look up secret runewords, such as Death.

I have seen hellforge hammer
in a Razor's Edge form... (Razor's Edge is a uniq item you cant find the same "picture" for an axe in elite exceptional or normal axes I looked in arreat summit)
no lvl required you can sock it and put something in
the only add it had was the same thing normal flails got (something demon dmg?)

go to op xbaalingx

and try make contact cleansing
he is the one that does blue-baal runs

if you find him on (he use his bot alot)
he will probaly show it to you

You can upgrade any quest item. Gidbinn and Malus just make a unique hand axe with no mods (not sure why the malus does that). Hellforge Hammer just turns into an axe, Horadric Staff turns into a hand axe (and keeps mods), I don't think you can up the flail, pretty sure I tried (or maybe it was Khalim's Will - 1 of the 2 isn't upgradeable, not sure about the other). This is not the same as an upgraded quest item, this is basically a bugged quest item.
