normal cows, tombs, etc are popular. For example, level 30 req affixes want level mlvl 40 areas. Normal baal runs do this pretty efficiently

http://classic.battle.net/diablo2exp/monsters/In terms of what you said about people targeting particular ilvl's, this is correct but a lot of these godly eth/reps and insane items come from dupers who run reroll scripts for a particular base item, they can reroll repeatedly and greatly increase their odds, then they get bought, passed around and traded and people lose track of the origin.

There is no such thing as an import method, there hasn't been since 1.10
