Here is what happened: The game picked Monarch as the base item, and the set quality check was successful.
However, the Monarch list will always be empty (there is no Monarch in the game) and so the game generated a replacement Monarch with 200% (double) durability.

This is known as a failed Monarch (Monarch Durability: 156 of 172 = 200%·(78 of 86)), and it is not glitched.
