if he was born in iran and was granted german citizenship as a refugee he would hold only german citizenship from the german perspective
but iran (like some other countries) doesn't allow citizens to renounce their citizenship, so from an iranian perspective he would still be an iranian citizen (*)
there are some special considerations for children born in germany to non-eu (or swiss) parents, most of them have to decide on their citizenship by age 23
(*) in those cases where people keep the citizenship of their origin the german authorities would advise the person that they cannot assist if the person travels to said country and has troubles there

let me add that those brits who voted 'remain' and want german citizenship better hurry - but to take another eu citizenship might be more viable so that they have not to renounce the british citizenship in the future
