One bag of semi-sweet chocolate morsels (chocolate chips)
One can of "Nestle's" La Leche sweet and condensed milk

Put both into a microwavable safe bowl.
Microwave, 1-2 minutes, mixing in 30-second intervals
Put a sheet of parchment paper into a casserole-style dish
Spread your mixture evenly into the dish, and refrigerate for about an hour
Cut, plate, and enjoy :drool:

But if you use those ingredients, you are not making it from scratch, which I think is what OP wants.

OP should have been more clear on intentions and desires.
I gave a simple recipe to use because it appears as if OP may not be highly experienced in the field of cooking
