I'm in chaos and out and sometimes doesn't activate

corpses without flesh and bones in them can't make grim wards
you can't make a skull totem out of animated suits of armor, and there's no flesh and blood on skeletons to decorate it
grim ward cannot target the corpses of doom knights, bone fetishes, skeletons, skeleton archers, skeleton mages, finger mages, wraiths, willowisps, swarms, tentacle beast tentacles, gargoyle traps or blood hawk nests.

Grim ward sucks anyways, not only does it not break immunities like amp/decrep does, it doesn’t stack with their damage either.

Unless you don’t have access to those, don’t use it.
