Yeah, once you kill the very last monster in Hell mode a portal opens. You enter and Tyrael is there, he'll bang on about how great you are, etc, then he gives you 5 random runes. Lowest being Lo.

It's been a well kept secret for many years, because the people who knew about it wanted to profit from it.
