EVERYTHING EXCEPT SUPERUNIQUES:

Once you know the alvl, it's pretty simple:

regular mobs: mlvl=alvl
champions: mlvl=alvl+2
uniques & minions: mlvl=alvl+3

SUPERUNIQUES:

Those have a fixed mlvl (not based on alvl):

andariel: 12|37|62
duriel: 22|47|72
mephisto: 26|51|76
diablo: 40|65|90
other superuniques: data missing

i don't think mlvl follows alvl in classic, e.g. special urds are mlvl 85?

They do, at least for everything randomly generated (superuniques are the only exception).
It's just that alvls are different (what SIVIART posted is for LOD, check the link in my previous post for classic alvls).
Regarding urdars, the highest mlvl they can have is 77|79|80 (regular | champions | uniques & minions).
