+1 char skills was rLvl 22 in patch 1.04. so there are many rlvl 22 ammys with +1 skills and other godly mods.

+2 was higher than 19 in all patches, and Imp brow was either some wonky creation of a flaw in the engine or an import when the open > closed type packet manipulation was still open.

but ill stop spamming now, bholds are really the only duped rares. plenty of duped jewels like Bone Eye tho.
