It's relatively smooth, but iirc the drops are split between the 2 players as the items have player tags on them, but both players can see them. It's kind of a bear, and when one person pauses to go through their inventory it pauses the game for all players until that person is done with their actions. Additionally you can't leave the same screen :(
This may have changed as it's been a while, but that's what I remember. Wasn't a huge fan of the couch co-op.
Thank you for the insight that sounds terrible. One of my favorite things about diablo was lootsharing and if the interface isn’t set up so you can still walk around while they loot is just sad...
