Having the game reference a txt file isn't typically considered either a mod or a hack. All of the rune words are available in the game files themselves, but you typically have to be on a battle net server to access them. The txt file bypasses it.

Now, if I said, "create a whole bunch of new rune words that do silly things." That would be a hack.

The way you enable those runewords is called modding. You can also put itemratio.txt there with increased unique chance.
