Once you put a runeword in it, it stops being an ith. Hybrid runewords are not iths so an ith checker will not see it. The game has an ith scanner that checks whenever you leave a game.
Unless you actually think the server should be checking the contents of everyone's inventory 25 times a second and flagging them for deletion, which could be postponed indefinitely by just perming out frame-perfect with a bot
The ith scanner will delete items in more than just save & exit events. Taking a waypoint or closing a trade window are two examples. I dunno why they can't make an update to the existing ith scanner that verifies rune combos & runeword bonuses server-side. Maybe someone wants to point out the difficulties associated with doing so.
Another bug (potentially related to this WW one, unless someone wants to explain why it isn't) is with casting inferno, arctic blast, and potentially blade fury (haven't tested yet). If you alternate between casting one of these and moving rapidly, you will soon find that you are rubberbanding to your original position (I believe this is because since 3/29/2018 the game server is less forgiving about canceling animations - and the animations on those skills are plenty lengthy). Although you are pretty defenseless while performing this, it allows you to steal namelocks, make huge teleports, or cast various other coordinate based skills like Blizzard at impossible distances. Not really any point if you have a resolution hack, though. You can also get the same results if you skill bug with charge or whirlwind since you'll be moving on your screen but not on others'. Since it basically lets you scope out an area from far away, I've been calling these kind of tricks astral projection
