You can't target farm it anymore.
It's in the general unique pool so any boss can drop it.

Greater Lair bosses in T12 are more likely to drop it.

You really can't. The items in the general unique pool hardly drop (at least for me). I ended up trading for one after probably 500+ boss runs.

For those world drops just buy them, it’s so ass trying to farm them especially if you want a good one
