from my experience with dealing with mistreated dogs that piddle when you reach towards them, it's more than likely a sign of fear that they're gonna get abused again.
it's like PTSD for the little guy.
the best thing your brother can do is show him love everyday and take him on walks in a routine manner.
slowly socialize him to other people that will treat him kindly too.
he might have been abused by the leash in some way so talk happy and in a high pitched voice while putting on the leash too.
it takes time for a dog to get over a bad past (if they ever get over it at all)
worried about the carpet?
i had a dog that piddled when i reached towards her so instead of reaching to put the leash on i would call her to the door first and let her outside the door to put her leash on and it really helped her to not piddle on the carpet. :)
