Twitch and grim shield is cheap and under rated. Hsarus set is under rated(40% bonus to ar and 120 more defense than war belt and toes). There is no getting around a gg weapon though.

with 4k hp goblin toes average damage does an average of 32 damage a swing to death. A nice bhammer will do an average of about 150 per hit to death. A barb with hsarus has a chance to hit of about 19% vs. defense barb and 15% chance to be hit. That is a 25% increase in chance to hit which is more than goblin toes damage. twitch, hsarus, grim helm, war gaunts, hsarus has 15k ar 6.5kd and 75 block. It is cheap and will land about 25% more hits against a nice defense barb with 11k ar. Basicly the extra damage of goblin toes is made up by hitting them more with a high damage weapon. The lower damage weapons rely on crushing blow more. Clegs can use sigs cage and toe and hit 18k ar and kill with massive crushing blow. The more strength you put into a barb the less crushing blow matters as well. Hsarus, grim shield, twitch, grim helm, war gaunts, and a gg bhammer can keep up with a fully decked out defense barb. If going exec or naga i would go sigs with war belt grim shield and toes.
