What do you mean healthy? Some must avoid excess calories while others seek fat and protein.
I would start with tuna, maybe throw some tomato and sharp cheddar cheese. Slap a nice piece of lettuce on there.
For bread I'd use some whole wheat bread that is low in sugar (no syrups) and high in protein.
I want high protein with low calories and a good amount of fiber.
