No. Whoever is in most serious need should get it. If both are equally ill then whichever came first.

So if I get liver disease from booze, don't stop drinking, and crash my liver worse than a kid who has hep c, I should get the liver?

easy, the people who decide who gets the liver
whether they choose a random lottery system, first come first serve etc. it is up to them though i'd favor a merit based system

No, if they are equally ill then there is an obvious choice based on future life expectancy and likelihood to have further complications with the new organ either due to their health situation or lifestyle.
