Did he buy the building outright? If not he will have a large rent expense.

Gyms themselves make very little additional profit that can be paid through a bonus or or paid in dividends from memberships that anyone from a day job wouldn't make. Certainly not from the ones iv seen anyway. Large franchise chain gyms make their mint by not having a cap, most have 1000s of members but only 100s of active members. The money is made on people who buy the pass but go twice.
A gym will not make money off the people who go. Its the people who don't.
The gym itself is beautiful don't get me wrong. But revenue is not a bottom line and 650K in revenue is dust in the wind if he rents. Unless rental prices are drastically different in the states.
